What forms do I need to fill out before my first visit?

Before your first visit it is critical to review all materials presented to you and make sure that your medical files have been sent over.  This will allow your doctor to better communicate with your other providers and understand your medical history for proper diagnosis.  Filling out all the forms in advance will greatly decrease the wait time for your initial visit.  These forms include:

  • Electronic Communication Consent Form
  • Authorization to receive medical information
  • IVF Screening Tests-Acknowledgment
  • Male Partner Information Questionnaire
  • Personal Information/Social History
  • Gynecological History
  • General Medical Information Disclosure
  • HIPAA Consent Form
  • HIPAA Notice of Privacy Policies
  • Primary Insurance
  • General Patient Registration

On a case by case basis there may be additional forms to fill out such as Embryo/Egg Disposition forms and other documents relating to retrievals and storage.
